Jack Rabbit
Jack Rabbit derailed and injured three riders on August 5, 1998.  It was traveling too fast for a turn after passing through three sets of brakes.  Since the coaster was built before trains were locked to the rails with up-stop wheels under the tracks, it was able to leave the tracks.
